ONE PROGRAM RETAINER FEE PAYS FOR ENTIRE TREATMENT PROGRAM MEDICAL SERVICES AND PRESCRIBED MEDICATIONS DISPENSED DIRECTLY TO PATIENT BY AN
INDEPENDENT PHARMACY

National Medical Clinic, Inc. ("NMC") is a national patient medical services management corporation with access to physicians located in a number of states. NMC contracts with physicians, diagnostic medical laboratories, laboratory services resellers and other service providers to determine the total medical treatment program fee for each treatment offered. This includes the cost for searching and locating a physician in an area where there is no NMC network physician to provide a patient the treatment sought.

NMC offers medical treatment programs for a single fixed medical retainer fee for services to be provided a patient by NMC and its medical services contractors during the selected medical treatment program. The Medical Treatment Program fee for each medical treatment program includes the cost for a medically appropriate quantity of medication prescribed by the physicians and dispensed to patients by an independent U.S. pharmacy with regard to the medical treatment program selected.

The only additional costs that may be incurred by the patient in excess of the program fee is for additional medication prescribed by the patient's treating physician during the course of treatment when indicated by the patient's response to treatment; and the cost of any additional medical laboratory diagnostic tests ordered by the treating physician during the course of treatment with the consent of the patient. The patient may seek reimbursement of all, or a portion of, the Medical Treatment Program Fee paid to NMC by filing a claim for reimbursement with their medical insurer.

This process applies to all National Medical ™ treatment programs described on this site.

Enrollment and Referral to Physician

New patient registration with National Medical Clinic and request for enrollment In a medical treatment that the patient desires to undergo is conducted by telephone enrollment. No NMC medical treatment involves the prescribing or use of any controlled substance prescription drug. If an NMC physician qualified to treat patients for the treatment sought by the patient is located within a reasonable distance from the patient's location, then the patient may register and enroll in the treatment at the time the patient pays the retainer fee with a credit card. Patients register and enroll in an NMC medical treatment program by phoning NMC Patient Services at phone number (800) 582-0095.

Medical Retainer Fee Refund Policy

If the NMC physician declines to issue the prescribed treatment sought by the patient a physical examination and evaluation of the patient's medical complaint and condition, the patient's retainer fee is refunded less the fees and costs incurred by the patient in obtaining the physician's clinical assessment and evaluation. These costs include the fees and costs for the patient's physical examination, clinical assessment and evaluation by the physician, diagnostic testing and clinic services posted on the clinic website. The patient shall be treated if treatment is indicated and the sought treatment could be beneficial to treating the patient's medical condition.

Insurance Claim for Reimbursement with Patient's Medical Insurer

NMC does not accept or process medical Insurance claims, but it does provide the patient an Itemized statement of physician, diagnostic medical laboratory and clinic fees. The statement also includes the patient's cost for treatment medications dispensed directly to the patient by a U.S. pharmacy pursuant to any prescription issued by the physician following the physical examination and clinical assessment of the patient.
